    Polidistrectual vascular involvement in Familial Hyperchilomicronemia

    A 72-year-old man was referred to our clinic with a lipid profile, under combination therapy with a statin and ezetimibe, characterized by severe hypertriglyceridemia (7230 mg/dl), hypercholesterolemia (374 mg/dl), low HDL-cholesterol (17 mg/dl), and normal circulating Lp"a". The patient had undergone post-traumatic splenectomy and presented a history of systemic hypertension treated medically with well-controlled blood pressure. The patient had also presented in the past abdominal pain with subsequent diagnosis of chronic pancreatitis complicated by diabetes mellitus, well compensated when he came to our attention. Following a pathological exercise test, a coronary angiogram was performed which showed a significant stenosis of the right coronary artery, which was treated successfully with percutaneous coronary intervention. Clinical work-up revealed ectasia of the abdominal aorta (28 mm), non significant bilateral carotid artery disease, and peripheral artery disease of the femoral-popliteal axis symptomatic for intermittent claudication. A lipidogram was also performed and electrophoretic lipoprotein patterns did not vary 2 hours after heparin infusion, pointing to the existence of lipoprotein lipase deficit. Electrophoresis also showed a broadband of chylomicrons at baseline, at the beginning, and at the end of heparin infusion. Hyperchilomicronemia is a rare genetic disorder with an incidence of 1 per 1000000. Following diagnosis, our patient began plasma exchange therapy with subsequent improvement of his lipid profile. At the present time, he is regularly followed up at our clinic and non invasive imaging has excluded any significant progression of atherosclerosis after 2 years of therapy

    Perspectives of 2D and 3D mapping of atmospheric pollutants over urban areas by means of airborne DOAS spectrometers

We propose some innovative solutions in the field of DOAS (Differential Optical Absorption Spectroscopy) remote systems, utilizing diffuse solar light as the radiation source. We examine the numerous potentialities of minor gas slant column calculations, applying the «off-axis» methodology for collecting the diffuse solar radiation. One of these particular approaches, using measurements along horizontal paths, has already been tested with the spectrometer installed on board the Geophysica aircraft during stratospheric flights up to altitudes of 20 km. The theoretical basis of these new measurement techniques using DOAS remote sensing systems are delineated to assess whether low altitude flights can provide 2D and 3D pollution tomography over metropolitan areas. The 2D or 3D trace gas total column mapping could be used to investigate: i) transport and dispersion phenomena of air pollution, ii) photochemical process rates, iii) gas plume tomography, iv) minor gas vertical profiles into the Planetary Boundary Layer and v) minor gas flux divergence over a large area

    Cognitive impairment and polidistrectual atherosclerotic disease in chylomicronemia syndrome: A case report

    A case of chylomicronemia syndrome is reported in a 72-year-old male with distinctive features of chronic pancreatic damage, severe hypertriglyceridemia, polidistrectual athero- sclerosis and premature cognitive impairment. Although the patient had a positive history for recurrent episodes of pancreatitis the characteristic lesions of the hyperchylomicrone- mia syndrome, such as eruptive xanthomas and lipemia retinalis, were not present and splenomegaly could not be documented due to a previous post-traumatic splenectomy. Based on clinical phenotype, an apolipoprotein C-II deficiency was excluded by a fresh plasma infusion test, in which clarification of the patient plasma was not obtained. The absence of changes in the lipoprotein electrophoretic plasma after heparin infusion can be secondary to a lipoprotein lipase deficiency, a rare genetic disorder with an incidence of one per million. In relation to the resistance to diet and drugs, plasma exchange therapy was performed. After 3 years of this treatment there was no significant progression of atherosclerosis

    Microbiological risk in operating rooms: new strategies for infections surveillance

    In this paper the authors describe the program of epidemiological overseeing of the surgical site infections activated in the operating department of the Cento Hospital (Ferrara). The purpose of the program is to systematically collect the data related to the frequency and distribution of the infections contracted during a surgical intervention and to analyze the environmental factors correlated to them, the concentration of particles and of UFC, the characteristics of the ventilation system, the number of people in the room, the type of clothing, the systems of cleaning and disinfection, the type of intervention etc. The objective of the research consists of identifying the actions for prevention of the infections and to get an indicator of the quality of the surgical activities. Since the greatest obstacle to this kind of activity is related to the man-hour cost far the information collection, researchers of the University of Ferrara and the Cento hospital have setup an electronic system of automatic acquisition of the field data using computer technologies of different types (RFID-Radio Frequency IDentification sensor, PC pocket, net computer and programs). The management of continuous relief of the pollution level established in the operating rooms during every intervention, the automatic acquisitions of the main environmental parameters (temperature, relative humidity, instant air flow rate of climatization plant, state of the doors, level of pressurization of the room, level of stoppage of the absolute filters etc.) and of the clinical overview of the patient allow detailed knowledge of the conditions in which every surgical intervention is developed. Particular attention is paid to the state of hypothermia of the patient that is monitored in continuously before, during and after the operation, since according to some authors this determines an increase in the probability of contamination of the wound. The surgical wound is controlled daily in the hospital up to the moment of patient release and every three months for one year from the intervention, so as to record possible complication. This adopted strategy allows to verification in the respect of the behavioral protocols of the medical and nursing personnel
